*Fees for consultation and collaboration include comprehensive medical history, medical record consolidation and review, lab and imaging test ordering and interpretation, and lifestyle modification recommendations and monitoring. Fees for obtaining the labs, imaging, and other outside testing are not included and paid directly to the third party.
